At a minimum, we need to know the "Final Diagnosis", in order to explain the findings to you. All identifying information should be removed. We will then review the submitted materials, and provide you with an explanation of key components of your report, as well answers to any specific questions.
Copying and pasting the diagnosis and any other pertinent fields works best. Alternatively, blacking out the name and date of birth of the patient at the top of the report or a screen shot of the Final Diagnosis, is sufficient. This will help de-identify any protected health information.
